Treatment is by appointment only. Treatments can range from 30-90 minutes.

Wear comfortable/loose clothing.

Gym or exercise attire is often appropriate.

Bring your insurance information.

Information forms require approximately 15 minutes to complete.

For your convenience, you may download the form below and complete prior to attending.

Referrals, pre-authorization/certification, are often required. Discuss with your primary care doctor as needed. We can often answer your questions concerning insurance requirements.

In New Jersey, a prescription for physical therapy is not necessary, however many insurers have referral or pre-certification requirements. Further information about specific services can be obtained by calling our office. If necessary, a therapist can discuss treatment options with you prior to attending.
